I always reach for quality Italian sausage with a good balance of herbs and spices because that’s where much of the dish’s character comes from. Fresh garlic and a splash of cream give it that luscious texture I love. Choosing the right pasta is also important; fettuccine’s wide ribbons hold the sauce beautifully, creating a perfect bite every time.
- 12 ounces fettuccine pasta
- 1 pound Italian sausage (mild or spicy, depending on preference)
- 2 tablespoons olive oil
- 3 cloves garlic, minced
- 1 small onion, finely chopped
- 1 cup heavy cream
- 1/2 cup grated Parmesan cheese
- 1/2 cup chicken broth or white wine
- Salt and freshly ground black pepper, to taste
- Fresh parsley, chopped (optional, for garnish)
Substitutions: For a lighter version, you can swap heavy cream for half-and-half or a mixture of milk and cream cheese. If you prefer gluten-free, rice or corn-based fettuccine works well too.
Instructions
- Bring a large pot of salted water to a boil and cook the fettuccine until al dente, about 10-12 minutes. Drain and set aside, reserving 1/2 cup of pasta water.
- While the pasta cooks, heat olive oil in a large skillet over medium heat. Add the Italian sausage, breaking it up with a spoon, and cook until browned and cooked through, about 6-8 minutes.
- Add the chopped onion and sauté until softened, about 3 minutes. Stir in minced garlic and cook for another 30 seconds until fragrant.
- Pour in the chicken broth or white wine, scraping any browned bits from the bottom of the pan. Let it simmer for 2-3 minutes to reduce slightly.
- Lower the heat and stir in the heavy cream and Parmesan cheese. Let the sauce simmer gently until it thickens, about 4-5 minutes. Season with salt and pepper to taste.
- Add the cooked fettuccine to the skillet and toss to coat evenly with the sauce. If the sauce feels too thick, add reserved pasta water a little at a time until you reach your desired consistency.
- Remove from heat, garnish with fresh parsley if desired, and serve immediately.
